One of child’s parent was a U.S. citizen at the time of child’s birth, and that U.S. citizen parent must have been physically present in the U.S. for 5 years, at least 2 of which were after the age of 14

You may replace a lost, mutilated, destroyed Certificate of Citizenship or if your name has been changed and you need the certificate in the new name.
